Seen on YouTube, on the Apple Demo channel, a PVT Apple Watch prototype running the test OS.
It operates under SwitchBoard (a system dedicated to testing, different from watchOS or iOS) and is obviously in very poor condition. It has been patched up just to boot, which is already an achievement in itself. Several applications designed for testing the watch can be seen, featuring an interface very different from watchOS.
